Smoking, Bidi, Gutka and Erectile Dysfunction: What Tobacco Does and What Improves When You Quit

Published 13 Jun 2024 · Updated 2 Oct 2026 · 9 min read

Short answer

Yes, tobacco can cause erectile dysfunction. Nicotine tightens blood vessels and the chemicals in smoke damage their inner lining, so less blood reaches the penis. Smokeless tobacco such as gutka and khaini is not a safe alternative. The encouraging part: in many men, erections improve after quitting, usually over weeks to months.

Does smoking cause erectile dysfunction?

Yes. Smoking is one of the best-established lifestyle causes of erectile dysfunction (ED), and men who smoke are noticeably more likely to have ED than men who have never smoked. The risk tends to rise with how much and how long you smoke.

It is also one of the more hopeful causes, because tobacco is something you can change. New to the topic? Start with our guide on erectile dysfunction: causes, symptoms and treatment.

How do nicotine and tobacco damage blood flow to the penis?

An erection is mainly a blood-flow event, and tobacco attacks blood flow in two ways: nicotine squeezes the vessels right now, and smoke slowly injures their lining over the years.

  1. Nicotine and smoke chemicals enter the blood
  2. Vessel lining (endothelium) is injured
  3. Less nitric oxide is available
  4. Arteries stay narrower and stiffer
  5. Less blood fills the penis
  6. Weaker or shorter-lasting erection
  • Nicotine narrows blood vessels. Each cigarette or bidi briefly tightens arteries and raises heart rate and blood pressure. The penis has very small arteries, so it feels this early.
  • The endothelium gets damaged. The endothelium is the thin lining of your blood vessels. It releases nitric oxide, the chemical that relaxes the arteries so blood can rush into the penis. Smoke chemicals reduce nitric oxide and leave the lining inflamed.
  • Plaque builds up. Long-term tobacco use speeds up fatty deposits in arteries (atherosclerosis).
  • Blood pressure and sugar worsen. Smoking makes high blood pressure and diabetes harder to control, and both are major ED causes. See our guides on high blood pressure and ED and diabetes and ED.

Because the same arteries feed your heart, brain and penis, new ED in a man over 40 is a reason to check heart risk, not just a bedroom problem. ED can appear roughly 3 to 5 years before a heart attack or stroke.

Are bidi and cigarettes equally harmful for erections?

All smoked tobacco harms blood vessels, and none of the common forms in India is a safe choice. Bidis still deliver nicotine, tar and carbon monoxide.

Tobacco forms common in India and what they do
FormDelivers nicotine?Main concern for erections and health
CigaretteYesEndothelial damage, plaque, carbon monoxide, lung and heart disease
BidiYesSame as cigarettes, often with more puffs per stick; high tar and carbon monoxide
Gutka / pan masala with tobaccoYesNicotine and other chemicals absorbed through the mouth; strongly linked with oral cancer
Khaini / zarda / chewing tobaccoYesNicotine kept in the mouth for long; oral cancer and gum disease; vessel effects from nicotine
E-cigarette / vapeUsuallyNicotine still tightens vessels; long-term effects are not yet well known

Is smokeless tobacco like gutka and khaini safer for erections?

No. Smokeless tobacco is not a safe substitute for smoking, and gutka or khaini should not be seen as a "harmless" habit that spares your sex life.

The nicotine in gutka, khaini and zarda is absorbed through the lining of the mouth, goes into the blood and reaches the same vessels as nicotine from a cigarette. Because the pouch sits in the cheek for a long time, nicotine keeps being absorbed for longer. Research linking smokeless tobacco directly to ED is smaller than for smoking, so we do not claim the effect is identical. But the vessel effects of nicotine are well known, and smokeless tobacco brings its own serious harm: mouth cancer, gum disease, tooth loss and mouth scarring (submucous fibrosis).

Does tobacco affect sperm quality and fertility?

Yes. Smoking is linked with lower sperm count, weaker sperm movement (motility) and more abnormally shaped sperm, and it can also damage the DNA inside sperm.

Smoke toxins increase oxidative stress in semen. Evidence on gutka and khaini and sperm is thinner, but the sensible advice is the same: stop all tobacco if you are trying for a baby. Sperm take about 2.5 to 3 months to mature, so improvements show up in a semen test about three months after quitting, not the next week. Read more in our guide to low sperm count and motility.

What improves after you quit tobacco, and roughly when?

Some benefits begin within hours, while erections and sperm take weeks to months to recover. Men who have smoked for decades, or who also have diabetes or high blood pressure, may need more time and extra treatment.

A rough recovery timeline after quitting (individual results vary)
WhenWhat typically changes
Within hours to 2 daysHeart rate and blood pressure settle; carbon monoxide leaves the blood
First 1 to 2 weeksCravings peak in the first days, then ease; taste, smell and sleep often improve
1 to 3 monthsCirculation improves and the vessel lining begins to recover; some men notice firmer or more reliable erections
About 3 monthsA new batch of sperm has matured, so semen quality can be re-checked
6 to 12 monthsMany men with smoking-related ED see a clear improvement; heart risk keeps falling over the years

Quitting does not promise that ED will fully go away. If erections are still weak after a few months, another cause such as diabetes, low testosterone, stress or a medicine side effect may be involved. Tablets such as sildenafil or tadalafil can be added, only on a doctor's prescription.

What about vaping and e-cigarettes?

Vaping is not a safe way to protect your erections. Most vapes deliver nicotine, and nicotine tightens blood vessels and raises heart rate and blood pressure just as it does from a cigarette.

E-cigarettes avoid some smoke chemicals, but long-term effects on erections and sperm are still being studied. Switching to a vape is not the same as quitting nicotine. In India, e-cigarettes are banned for sale and production, so what you buy is unregulated and you cannot be sure what is in it.

How can you quit tobacco in India?

Most men do best with a plan, support and, when needed, medicines, rather than willpower alone. Nicotine dependence is a medical condition, so needing a few attempts is normal.

  1. Pick a quit date within the next 1 to 2 weeks, and tell one person you trust. Remove cigarettes, bidis, lighters, gutka sachets and khaini pouches from your bag, bike box and desk.
  2. Know your triggers. For many Indian men: morning chai, after meals, late-night shifts, long drives, work stress and "just one" with friends at the paan shop. Plan a swap: a walk, water, saunf, a clove or sugar-free gum.
  3. Call the National Tobacco Quit Line: 1800-11-2356. It is a toll-free Government of India helpline that offers counselling to people who want to quit tobacco.
  4. Ask about nicotine replacement therapy (NRT). Patches, gum and lozenges give a controlled dose of nicotine without smoke and ease cravings. Your doctor can advise the strength and how to taper.
  5. Ask about prescription quit medicines. Doctors sometimes prescribe medicines such as varenicline or bupropion, only on a doctor's prescription and after checking your health and other medicines.
  6. Plan for slips. A slip is not a failure. Note the trigger and restart the same day.

What else helps your erections while you quit?

  • Move daily: about 30 minutes of brisk walking, cycling or jogging most days.
  • Eat for your arteries: dal, vegetables, fruit, nuts, curd and whole grains; fewer fried snacks (samosa, pakora), mithai and sugary chai. Vegetarian diets work well here.
  • Cut back on alcohol: it often goes with smoking and also weakens erections. See alcohol and ED.
  • Sleep and stress: night-shift workers often smoke to stay awake, so fix sleep timings where you can. Read about stress, ED and PE.

When should you see a doctor?

See a doctor if you use tobacco in any form and have erection problems that last more than about three weeks to a month, or if you cannot quit on your own.

  • You are over 40 and have new ED, even if you feel otherwise healthy.
  • You have chest pain or breathlessness; see a doctor urgently.
  • A white or red patch, an ulcer that does not heal for more than 2 weeks, or difficulty opening your mouth if you chew tobacco.
  • You are trying for a baby and have not conceived after a year.
  • You have tried to quit more than once and slipped back.

An online consult is a private first step: a doctor can check sugar, blood pressure, cholesterol and testosterone and help with quitting. If ED and early ejaculation both bother you, see ED and PE together.

Myth vs fact

Myth Gutka and khaini are safe because I don't smoke.

Fact Smokeless tobacco delivers nicotine and many harmful chemicals. It is a leading cause of oral cancer in India, and nicotine affects blood vessels no matter how it enters the body.

Myth Bidi is milder than a cigarette.

Fact Bidis deliver nicotine, tar and carbon monoxide, and are not milder in any way that matters. They are not a lighter option for your heart or your erections.

Myth The damage is already done, so quitting won't help.

Fact Blood vessels and sperm production can recover to a useful degree after quitting, and many men notice better erections within months. The earlier you quit, the better the outcome.

Myth Switching to a vape means I have quit.

Fact Most vapes still contain nicotine, which tightens blood vessels. They may reduce some smoke exposure, but your goal should be to come off nicotine altogether.

Frequently asked questions

Can ED from smoking be reversed?

In many men, yes, at least partly. Erections often improve over weeks to months after quitting because blood vessels recover. If diabetes, high blood pressure or low testosterone are also present, more treatment may be needed.

How long after quitting smoking do erections improve?

Some men notice a change within one to three months, and more improvement is common over six to twelve months. It varies with how long and how heavily you smoked and with your general health.

Does chewing gutka or khaini cause erectile dysfunction?

Smokeless tobacco delivers nicotine, which narrows blood vessels, so it is reasonable to expect harm to erections, though the research is less extensive than for smoking. Its clearest harm is oral cancer, gum disease and mouth scarring, which is reason enough to stop.

Will quitting tobacco improve my sperm count?

It can. Smoking is linked with lower sperm count, motility and normal shape, and sperm take about three months to renew. If you are trying for a baby, a semen analysis about three months after quitting is a sensible check.

What is the National Tobacco Quit Line number in India?

It is 1800-11-2356, a toll-free number set up by the Government of India. Counsellors help people who want to quit tobacco, smokers and chewers alike.

Can I use nicotine gum or patches to quit if I have ED?

Nicotine replacement is generally a safer route than continuing to smoke, because it removes smoke chemicals and lets you taper. Speak with a doctor first, especially if you have heart disease or high blood pressure.
